There are times in life where we find ourselves or our loved ones in a medical crisis where specialized services are needed after hospitalization. At Rose Garden we recognize that this can be a frightening experience that is often unexpected. Rest assured our professional staff is here to help both individuals and families to cope during this difficult time and to provide rehabilitative services with the goal to return home as soon as is medically indicated.

Melissa Harker, MSN is the clinical coordinator who oversees all sub-acute services. Melissa will meet with you upon admission to discuss the plan of care that will be provided to your loved one. Each person receives a specialized plan of care aimed at obtaining the highest quality of life both during and after a stay at Rose Garden.
Physical, occupational and speech therapy are offered on-site in our state of the art rehabilitation center. The rehabilitation department is overseen weekly by an onsite physiatrist.
Discharge planning is provided by Danny Glazer, MSW. Danny will work closely with you throughout your loved ones stay and arrange for any services that will be needed when it is time to return home.
